Seems like a classic "cold water/involuntary gasp" event which is why life jackets are emphasized for boating. This poor father.
This is taught during boating licencing courses and was part of receiving our kayaks. People falling into cold water involuntarily gasp and drown pretty well immediately.

My wife and I have a tandem kayak. We figure we have too much going on with the tandem so we wear life jackets. When we bring Ana, she wears a life jacket too.
